Most Painful Piercings Ranked (By Someone Who’s Done Them All)

If you’ve ever Googled “what’s the most painful piercing?” you’re not alone. Pain is the number one thing first-timers worry about — and fair play, nobody likes the idea of unnecessary agony. If you want to know the most painful piercings clients ask for, buckle up — here’s the good, the bad, and the ouch.

The truth? Every piercing has its own spot on the piercing pain scale, and everyone experiences it differently. That said, after 30 years behind the needle (and a fair few piercings in my own skin), I can confidently rank the most common piercings from “barely noticed it” to “saw the face of God.”

How We’re Ranking Pain

Before we start, here’s the most painful piercings system:

1/10 – You’ll wonder if I actually did anything.

5/10 – Sharp but manageable, maybe a watery eye or two.

10/10 – A religious experience you didn’t sign up for.

Least Painful Piercings

Earlobes – 1/10

Let’s be honest, this one barely counts. Quick, easy, and over before you’ve had time to flinch. Perfect for kids, first-timers, or anyone who wants jewellery without drama.

Tip: They heal fast too, but still follow aftercare. Check piercing care guide for the full rundown.

Eyebrow – 3/10

Looks cool, heals fast, and pain-wise? A little nip, nothing more. You might tear up (your eye’s reflex, not your courage).

Tip: Avoid heavy jewellery at first — lighter bars mean less irritation.

Nostril – 4/10

A quick pinch followed by one dramatic tear rolling down your cheek like you’ve just finished watching a sad film. Pain’s minimal, but it can be awkward if you knock it washing your face.

Tip: Don’t smother it in foundation straight after. Trust me, I’ve seen the rashes.

Mid-Level Pain Piercings

Tragus – 5/10

That little flap of cartilage near your ear canal? Piercing it feels sharper than a lobe, but it’s still over in a flash. The biggest annoyance is healing — headphones can irritate it.

Tip: A travel pillow can save your sanity if you sleep on your side.

Septum – 5/10

The dreaded “bull ring.” People think it’s the end of the world — it’s not. It’s sharp, makes your eyes water, but pain’s short-lived. Earning it a Mid rep on the most painful piercings.

Tip: Make sure your piercer hits the “sweet spot” (the thin bit of tissue). A crooked septum piercing can feel worse than it should.

Conch – 6/10

Right in the middle of your ear’s cartilage. It’s a deeper pinch than a lobe, but not unbearable. Healing can be slow, so patience is key.

Tip: Avoid pressing headphones or sleeping directly on it.

Navel (Belly Button) – 6/10

More pressure than pain. Some describe it as “weird” rather than painful. Healing takes a while though, so don’t rush it.

Tip: Loose clothing is your best friend. Tight jeans and fresh navels don’t mix.

The “Saw the Face of God” Piercings

Industrial – 7/10

Two holes, one bar, lots of commitment. Pain’s sharper and lasts longer since it’s double the fun. Healing is also a marathon.

Tip: Keep it clean and resist fiddling. Trust me, an irritated industrial is no picnic.

Nipple – 8/10

Yes, it’s as bad as you think. Sharp, intense, and lingers for a few seconds. The payoff? Looks great once healed.

Tip: Soft clothing and patience. And maybe avoid hugs from overenthusiastic mates for a bit.

Tongue – 8/10

Quick to pierce, but the swelling after? That’s where the pain sneaks in. Talking, eating, and even laughing can feel odd for a few days.

Tip: Stick to cold drinks and avoid hot curries straight away (you’ll only do it once).

Genital Piercings – 9–10/10

Yes, people do them. Yes, they hurt. But for some, the payoff is worth it. This isn’t for the faint-hearted, and definitely not a first-timer’s choice.

Tip: Only ever trust an experienced piercer. Cleanliness is non-negotiable here.
